1. Does the college pay for my athletic injury or illness?
- Our secondary insurance will cover athletic injuries from a supervised game or practice after your primary insurance has been billed. Our insurance does not cover unsupervised athletic injuries, non-athletic injuries or illnesses.
2. Do we need to contact anyone if we have a non-athletic injury or illness?
- Yes. You must contact the Athletic Training Staff. We will need documentation, especially if it will affect your participation status.
3. Can a non-athletic injury or illness that affects my participation status affect my eligibility?
- If we do not have documentation for any injury or illness that affects your participation, it could affect your eligibility in the long term.
4. What paperwork do I need to fill out as a new athlete? Does it need to be done yearly?
- All forms are listed under the Ursuline College Medical Clearance Checklist. There is a checklist for New/Transfer and Returning student-athletes. All forms and information are in the SWOL (SportsWare OnLine) See SWOL instruction form on how to access SWOL.
5. Can I participate in practices if not all of the student forms are completed?
- No, you will not be allowed to compete until all forms have been completely filled out and turned in to the Ursuline College Athletic Training Staff.
6. Can I get a physical with my own physician?
- No. You must get a physical with the Ursuline College team physician.
7. Is there any transportation available for medical appointments off-campus?
- No. You are responsible for finding transportation to your medical appointments. We may be able to assist in extenuating circumstances.
8. How does the secondary insurance work? Do I need to do anything?
- See UC Athletic Insurance Information. Report injury as soon as possible. You will receive a copy of the secondary insurance to take to physician appointment. All itemized medical bills, EOB's, and denials from your primary insurance must be turned in promptly. If the paperwork is not handled in a timely manner, then claims will not be paid.
9. Who should I contact if I sustain an athletic injury?
- You must contact a member of the Ursuline College Athletic Training Staff as soon as possible.
10. Who should I contact in an emergency and the athletic trainers' are not available?
- If an emergency occurs when an athletic trainer is not available, go to the emergency room or physician and bring back all documentation to the athletic training staff as soon as possible afterward.
11. Can I see my own physician for an athletic injury?
- Yes, as long as you receive approval from an Ursuline College Athletic Trainer. If prior approval is not obtained, then it will result in a loss of insurance coverage for any medical expenses incurred. Exceptions include any athletic related emergency in which the training room is closed, the athletic trainer is unavailable or you are traveling with a team and the athletic trainer is not present.
12. What is required prior to trying out for a sport?
- You must have had a physical at least 6 months prior to the try-out. This physical does not need to be done with our team physician. You must also provide us with documented results of a sickle cell solubility test. See the compliance coordinator and/or the head coach for further information and other forms that are required prior to the try-out.
